Geriatric Primary Care Administrative Coordinator

Position Summary

As the Geriatric Primary Care Administrative Coordinator, you will be the first point of contact for patients and caregivers and the behind-the-scenes organizer who keeps clinical operations running smoothly. You will balance front-desk hospitality with detailed care-coordination tasks—scheduling, closing preventive-care gaps, and maintaining crystal-clear documentation—so our clinicians can focus on delivering high-quality geriatric care.

Key Responsibilities

Patient Scheduling & Outreach

  • Schedule and confirm annual wellness visits, follow-ups, home visits and transition-of-care appointments.

  • Make reminder calls / send electronic notifications.

  • Coordinate specialty referrals and ensure timely appointments.

  • Assist patients and caregivers in navigating the patient portal.

Care-Gap & Risk-Adjustment Tracking

  • Monitor open preventive-care gaps (screenings, immunizations) and proactively reach out to address them.

  • Maintain accurate tracking of risk-adjustment diagnoses and flag follow-up needs for the clinical team.

Front Desk & Intake

  • Greet patients, answer phones, respond to inquiries, and relay messages promptly.

  • Verify insurance, collect and review intake paperwork, and administer screening questionnaires.

Clinical Documentation & EHR Management

  • Scan and index documents (labs, consult notes, ED/hospital reports) into the EHR.

  • Keep patient records up to date and error-free to support seamless care transitions.

  • Coordinate with clinicians, medical assistants, and care coordinators to maintain efficient patient flow.

  • Support EMR document management

Administrative Support

  • Assist with scheduling meetings, notes and coordinating follow up tasks.

Qualifications

Education

  • High-school diploma or equivalent required; medical-assistant training or certification preferred.

Experience

  • Prior work in a medical office or healthcare setting (geriatric experience strongly preferred).

  • Hands-on familiarity with electronic health record (EHR) systems.

Skills & Attributes

  • Exceptional organization, accuracy, and follow-through.

  • Clear, compassionate verbal and written communication.

  • Empathy and patience when working with older adults and their caregivers.

  • Understanding of primary-care workflows, preventive-care guidelines, and basic insurance procedures (or willingness to learn).

  • A current driver's license

  • Ability to speak Spanish would be helpful

Kronos Health, PLLC is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our growing family medicine primary care office. As a vital member of our collaborative healthcare team, you will play a key role in supporting patient wellness, driving clinical excellence, and helping to shape the future of primary care.

Key Responsibilities

Patient Care

  • Administer medications and provide basic wound care under clinician supervision

  • Understand and administer immunizations for all age groups

  • Educate patients on chronic disease management (e.g., diabetes, hypertension)

  • Triage patient calls and clinical concerns

  • Support coordination of care for patients

  • Record vital signs, reconcile medications, and update EMR (Athena)

  • Perform in-office procedures: EKGs, strep/flu/COVID tests, ear lavage, etc.Manage lab samples and documentation

Clinical Support & Admin

  • Support smooth office operations and patient intake

  • Assist with clerical tasks: documentation, scanning, correspondence

  • Maintain and stock medical supplies

  • Autoclave instruments and support vaccine supply management

  • Prior Authorization and communication with medical insurance

Quality & Leadership

  • Follow clinical protocols and participate in quality improvement initiatives

  • Help onboard new team members and foster a collaborative environment

  • Communicate clearly with patients, families, and team members

  • Identify and propose solutions to improve patient care

Requirements

  • Active Massachusetts RN or LPN license

  • 2+ years recent nursing experience (primary care preferred)

  • BLS certification required

  • Experience with AthenaHealth (preferred)

  • Bilingual abilities (Spanish) are a plus

  • Completion of accredited RN/LPN program
